About Nick Tolentino MBA, MSN, APRN

Guided by Experience, Committed to You

Nick is the founder of Coastal Balance Wellness, PLLC, a practice born from his vision to connect with patients differently—spending more time, truly listening, and addressing health at its root causes. He wanted to create a space where care is relationship-first, holistic, and individualized, reflecting his belief in what medicine should be.

Born and raised in Central Missouri, Nick enlisted in the U.S. Navy at 17, serving 14 years as an Independent Duty Corpsman (IDC)—a highly specialized, senior enlisted Navy Hospital Corpsman trained to operate as the sole medical provider in isolated, operational environments without direct physician supervision. His service in high-pressure and combat situations exposed him to harsh realities, shaping a profound empathy and understanding that allows him to connect with patients on a deeply human level.

After retiring from the Navy, Nick relocated to New Hampshire to serve as the administrative officer of mental health services for Veterans Affairs (New England Region). It is this experience that led him to testify before Congress to help shape the Mental Health Access to Continued Care and Enhancement of Support Services Act of 2012--which aimed at improving access to mental health services for veterans, service members, and military families.

Nick then transitioned to clinical practice, quickly advancing from RN to Nurse Practitioner at the University of New Hampshire, applying his military and healthcare experience to bring compassion, precision, and leadership to patient care. Over the past decade, he has worked extensively in internal and primary care, helping patients navigate complex medical and wellness challenges.

Nick draws on his own vulnerabilities to create a space where he can listen deeply, without judgment, and connect authentically with patients, understanding the complexities of their struggles. With Coastal Balance Wellness, Nick is embracing a new chapter in medicine—one where he can fully integrate functional medicine, hormone optimization, metabolic support, and peptide therapies, while building deep, meaningful relationships with his patients.

Unique Expertise: Serving Veterans & First Responders

Care That Understands the Demands of Service

Our care is grounded in awareness of the F-Star First Responder Protocol and delivered through a trauma-informed, evidence-based approach. Clinical decisions are informed by a deep understanding of the operational realities faced by first responders and veterans, guided by a provider who is also a combat veteran.

This lived experience, combined with functional medicine and current clinical evidence, allows for care that is both culturally competent and medically rigorous. We prioritize safety, trust, and autonomy while addressing service-related stress, exposure, and long-term health impacts with clarity and respect.

Serving Those Who Serve

Trauma-Informed Care

We provide care that recognizes the impact of service-related stress and trauma, prioritizing safety, trust, and patient autonomy in every interaction.

Understanding Service Culture

We draw on our provider’s experience as a veteran and first responder to deliver care that respects the realities of service and builds trust.

Evidence-Based Aproach

We use treatments and strategies that are supported by the latest research to ensure our care is effective and reliable.
